PASSPORT APPLICATION INFORMATION

All applications must be submitted in person. All children under age 16 must be accompanied by both parents. If parent and child have different last names, you must submit proof of your legal name change (e.g. Marriage Certificate or Court Order.) Document must be the original (photo copies will not be accepted). Please contact the County Clerk's office at 920-459-3003 for additional information or visit the National Passport website at http://travel.state.gov

Evidence of Identity - one of the following:
Valid Driver's License or State issued I.D.
Evidence of U.S. Citizenship - one of the following:
Certified Birth Certificate (printed on colored security paper or has an embossed seal)
U.S. Department of State requires the full names of the applicant's parent(s) to be listed on all certified birth certificates as primary proof of citizenship (submitted with application & returned when passport is issued). Naturalization or Citizenship Certificate (submitted with application & returned when passport is issued.)

Available in the County Clerk's Office - Fee $10.00

Passport Book:

Fees: (1) Check or Money Order payable to U.S. Dept. of State for $130.00* (separate check, or money order for each passport book application)

(1) Check or Money Order (or cash) payable to County Clerk for $35.00 counter fee for each passport book application (1 check, or money order can be written for all applicant counter fees and photos)

*Applicants under 16 yrs. of age - fee is $100.00 to U.S. Dept. of State

Timing: Allow up to 6 weeks for processing. Processing times only include the time your application is at one of our passport centers and do not include mailing times.

Expedited Passport Book:

Add an additional fee of $60.00 per passport to U.S. Dept. of State

Allow up to 3 weeks for processing.

Optional Postage for Passport Books Only: You may choose to pay an additional $21.36 for overnight postage when expediting a passport book from the passport agency to the applicant (this fee must be included in the check payable to U.S. Dept. of State). You may also choose to pay an additional $26.35 for overnight postage when expediting to the passport agency (this fee must be included in the check, money order, or cash payable to County Clerk).

Passport Card:

NOTE: The U.S. Passport Card may only be used for re-entry into the United States at land border-crossings and sea ports-of-entry from Canada, Mexico, the Caribbean, and Bermuda. Passport cards cannot be used for international travel by air.

Fees: (1) check or money order payable to U.S. Dept. of State for $30.00* (separate check, or money order, for each passport card application)

(1) check, or cash, payable to County clerk for $35.00 counter fee for each passport application

*For under 16 years of age - fee is $15.00 to U.S. Dept. of State

Timing: Allow up to 6 weeks for processing, or 3 weeks if Expedited. Processing times only include the time your application is at one of our passport centers and do not include mailing times.

Overnight delivery is not available for Passport Cards.

Applications must be signed in the County Clerk's office in front of the passport acceptance agent.

Passport Books & Cards are valid for 10 years if you are 16 years or older.

Passport Books & Cards are valid for 5 years if you are under 16 years of age.
