A person may be court ordered to search for employment and a job search form must be submitted to the Sheboygan County Child Support Agency.  There should be 5 job contacts per week listed on the form and the form needs to be turned into the Sheboygan County Child Support Agency on the last day of each month.  If job searches are being done for another program, such as Unemployment Insurance or W2 or Children First, a copy of the job contacts that are being sent to the other agency should also be sent to the Sheboygan County Child Support Agency.  If job contacts include Internet sites, document the site the application was made on this form and provide copies of any on-line application confirmations.  Failure to comply will result in further court action against you.
